One of the most important benefits for all AgeWell New York members is having a personal Care Manager to assess your needs and coordinate your services. We understand how complex it may be to access the right services, at the right time, and we are here to help.
Your Care Manager is a health care professional who will work with you to;
- Seek and coordinate solutions to meet your health and long term care needs.
- Create an individual care plan that will outline the services that are medically necessary for you.
- Provide authorization for you to start or continue receiving services, as well as make receiving these services as seamless as possible.
- Work with your Primary Care Physician and other health care providers to coordinate both your covered and non-covered services
