One of the most important benefits for all AgeWell New York members is having a personal Care Manager to assess your needs and coordinate your services. We understand how complex it may be to access the right services, at the right time, and we are here to help. Your Care Manager will assist in answering any questions you may have regarding the health care services you may need. Your Care Manager is a registered nurse or licensed social worker who will work with you to:
- Seek and coordinate solutions to meet your health and long term care needs
- Create an individual care plan that will outline the services that are medically necessary for you to receive
- Provide authorization for you to start or continue receiving services, and make the process as seamless as possible
- Work with your primary care physician and other health care providers to coordinate your covered and non-covered services.
